Mills Scholarship from the Foundation for Financial Service Professionals, I understand and accept the requirement that all funds granted me will be expended within the United States and that I shall be enrolled for a full or part-time degree program and shall pursue a course of study in the financial service field at a university within the United States or U.S. Territories. Further, I authorize the Foundation to use of my name and likeness to publicize the award. All information provided herein is true to the best of my knowledge.
